At Triple Point Global, we pride ourselves on a comprehensive and patient-centered approach to healthcare, driven by our unwavering commitment to excellence. Our dedicated team follows a carefully crafted Care Pathway that is designed to address the unique needs of each individual, ensuring that our services are tailored to provide the highest level of care. We understand that healthcare requirements can change over time, and we are committed to adapting our services to evolving needs, ensuring that our patients receive the best possible care at every stage of their journey.
How We Work
01
Full Individual Assessment
A thorough and all-encompassing evaluation of the client's needs is conducted in collaboration with their family, the primary nurse, and other healthcare experts. This process identifies all clinical requirements, staffing preferences, and training needs. The resulting detailed care plan empowers the individual to achieve their full potential and live the life they desire.
02
Recruitment plan
Qualified, skilled, and experienced staff are selected and their profiles are shared with the service user and/or family. Face-to-face meetings are scheduled with the shortlisted staff to address their care needs, as well as to cater to their social and mental wellbeing.
05
Specialist practical training
After conducting a thorough clinical evaluation of individual needs, we will pinpoint the necessary training to equip our staff with the confidence and competence to perform clinical procedures and to fully comprehend the client's condition. Conditions may encompass Motor Neurone Disease Awareness, Ventilation Support, Tracheostomy, Epilepsy, Percutaneous Endoscopic Gastrostomy (PEGS), enteral feeding, and Medication Administration.
06
Supervision and Integration
Our Complex Care staff receive continuous supervision to ensure they receive full support, share best practices, and maintain high levels of competency. This includes competency reassessments, scheduled visits, and surprise checks, all with the assistance of our clients.
03
Ongoing staff screening
A sustainable staff allocation plan is established to ensure consistent support in the event of staff absences. New team members are introduced to the client and their family before joining the care team.
04
Training
All employees are trained according to a specific set of standards that guide the conduct of health and social care workers in their daily professional activities.
07
Clinical support 24/7
We have a team of expert nurse advisors available around the clock to provide clinical advice and support.
08
Care commences
You can anticipate a comprehensive care plan assessment every 12 weeks, or more frequently if there are any changes. We will consistently assess the services provided to ensure a care plan that adjusts to your needs and grows with you.
